Requesting Care
Please Download and fill out the following form by clicking the link below and choosing SaveAs choosing an easy and convenient location in which to save the document.
- After you have downloaded this form. Open it up and fill out you details and save it.
- Then e-mail this document to us [as an attachment] at the address: wfdc@bigpond.net.au
To e-mail us go to our click this link wfdc@bigpond.net.au
- This will open your own PC's default e-mail program, with the correct address already entered for you.
- Add the Subject Heading "Request For Care"
- Select 'Add an Attachment', then select your completed "Request For Care" Form to add it to the e-mail, and click 'Send'
